ORI Survey
This survey is used to collect a complete health history (based on yourself, family members, and your surroundings).
Any personal information collected is encrypted before being processed to the data base systems.
All personal information is encrypted on this website system before it is sent including email.
The survey creates a unique 32 character id code that is used to collect the information.
At anytime you need to update or continue the survey, the id code must be entered on this screen to continue.
Once the code is generated, you can upload the key (as a text document) so that you can use it for future use.
In order to start the survey, you must accept the Recaptcha verification and press continue
This survey will not be accessible from mobile devices. The survey can be filled out on desktop, laptop, and tablet devices.

This email will include a link to continue the survey.
This code can't be resent to you (in any way) if you forget or lose the code.
